Our Agency offers a unique combination of premier home health care services such as skilled nursing, personal care, and therapy, to patients in the Miami Dade area. MDT Home Health Care Agency, Inc.’s mission is to provide quality services for the complete recovery or maximum rehabilitation of each patient.
Our mission and goals will be achieved by stressing efficiency in our operation, flexibility in providing services, keeping strict control on the quality of work rendered and compassion on delivering it. By maintaining a manageable patient census we are able to focus on providing the highest quality of care.

- To collaborate with the physician to develop an individualized plan of care that meets each patient’s needs in order to achieve their maximum health potential.
- To provide quality service by adhering to the agency’s philosophy, goals and objectives. The agency has a Quality Assurance program to ensure optimal service is provided to each patient.
- To encourage employee participation in educational opportunities. The Agency will provide monthly in-services and related workshops.
- To assist the patient and family in identifying their health care problems and needs.
- To actively participate and advise patient in community agencies and resources.
